Monthly Accountability Touchpoint
- 1 structured check-in (telehealth), 10–15 minutes, goal review & support
Medication Safety & Monitoring
- PDMP review when applicable, refill safety checks, UDS scheduling guidance (testing costs not included)
After-Hours Guidance
- Self-care instructions and crisis resource list (not an emergency service)
1 Clinician Visit / Month
- Telehealth medical or behavioral follow-up (up to 20–25 min)
- Medication review/adjustment (non-controlled), labs/orders as appropriate
- Brief counseling/relapse-prevention coaching
Care Plan Progression
- Documented progress notes and treatment plan updates aligned to SUD goals
1 Psychiatric Visit / Month
- Telepsychiatry (up to 30 min) for mood/anxiety/ADHD comorbidity management
MAT Service
- Evaluation & ongoing management for Medication-Assisted Treatment (e.g., buprenorphine)
- Induction/maintenance planning, side-effect & adherence support, coordination with therapy
- Refill cadence per clinical judgment and regulatory requirements

Our Policies
- Initial evaluation required before any prescriptions.
- Safe-use and PDMP checks precede controlled medication refills.
- UDS, check-ins, and visit frequency are part of safe MAT care.
- Lost/stolen medication is not routinely replaced.

Recurring monthly charge on sign-up date; prorated if upgrading mid-cycle.
No-Show Policy
24-hour cancellation window; missed visit counts as used. One courtesy reschedule/quarter. More than one is a $25 charge per occurrence.
